SUPPORTED GROUP LIVING RESIDENCES

Providing, developing and supporting access to a wide range of living options, including group homes and 24 hour supported apartment program.

 

SUPPORTED INDEPENDENT LIVING (SIL)

Designed to support adults who live in apartments in the community who require less than 24/7 support.

 

HOST FAMILY PROGRAM

This residential program provides family-based community living opportunities and is designed to enhance the range of options for a person, allowing their needs to be addressed in a more personal and flexible manner.

 

COMMUNITY PARTICIPATION SERVICES AND SUPPORTS 

Providing services and support co-ordination to families in the Port Colborne~Wainfleet area.

 

TRANSITIONAL AGED YOUTH PLANNING SERVICES

Providing assistance, training and support for youth (16-25) who are transitioning into adulthood

 

WITHOUT BORDERS

Programs that provide opportunities to participate in community recreational activities, social outings,

shopping, cooking and other leisure activities. 

 

PASSPORT PROGRAM 

Passport is a program that helps adults 18 years or older with a developmental disability to participate in their communities. It also helps caregivers of an adult with a developmental disability take a break from their caregiving responsibilities. The program is funded by the Ontario Government and administered by local Passport Agencies. When requested the association agency will assist families to hire trained support workers.
